Which funding types do you work with?

Altitude welcomes self-managed and plan-managed participants.

What areas do you support?

Our main service areas are Newcastle, Lake Macquarie, Maitland, Cessnock and the Hunter Valley, and Raymond Terrace.

What types of support do you provide?

Our services include community access, assistance with daily living, in-home support, one-to-one individual support, skill development, social and community participation, and support with transport and community activities where agreed.

How do I request support?

Use the Request Support form, call 0426 442 690, email admin@altitudesupportservices.com, or message Altitude on Facebook.

Can a support coordinator or family member make an enquiry?

Yes. An authorised referrer can use the participant referral form. We recommend keeping the first website enquiry brief and avoiding detailed sensitive health information.

Does submitting a form guarantee support?

No. An enquiry is the first step. Availability, suitability, worker matching and the support arrangement need to be discussed before services begin.

Is this an emergency service?

No. The website and enquiry channels are not emergency or crisis services.
